This is happening to me too on OSX 10.6.8 and it seems that this issue has been around for some time http://www.mail-archive.com/discuss-gnuradio@gnu.org/msg19013.html The dial_tone.cc example seems to works fine when set to 96kHz. I'm using gnuradio-core and gr-audio v3.4.0-113-gc491449c built without macports or any other ports.
