Dear Arther, i think you have to consider data element category since
you are talking about OPD and IPD.This can differ depending on their
categories. (Make sure they all have same category eg Age is the
same,Gender the same). Because most of the data element are the same but differ
when you see their categories.

Why not have a single data element for
Diabetes Mellitus
with category option combination
(Inpatient/Outpatient)/(ear infection acute, ear infection chronic etc etc etc)
and only have certain data element/category options in the IP form and
certain data element/category options in the OPD form?
