On one of the latest build upgrades, all my program stage data entry forms did not pull through. The dataentryform table still has entries for the Program stages but there's nothing in the front end. This problem started at the same time this error started showing up with Tomcat startup: * ERROR 2014-04-28 11:27:00,390 ERROR: constraint "fk_programstage_dataentryform" for relation "programstage" already exists (SchemaUpdate.java [pool- 2-thread-1]) * ERROR 2014-04-28 11:27:00,390 HHH000388: Unsuccessful: alter table trackedentityform add constraint fk_trackedentityform_dataentryformid foreign key (dataentryformid) references dataentryform (SchemaUpdate.java [pool-2-thread-1]) * ERROR 2014-04-28 11:27:00,390 ERROR: constraint "fk_trackedentityform_dataentryformid" for relation "trackedentityform" already exists (SchemaUpdate .java [pool-2-thread-1]) Any insight or input would be most appreciated.
