It uses the first answer it gets back from getaddrinfo I just scanned the man page. I didn't see anything about the order of returned answers. ntpd used to know if IPv4 and/or IPv6 was supported on this system. That feels like the sort of thing that Eric would have removed but I think I saw some of that code recently. -- These are my opinions.
