Cleanup the table indexes in ntp_control
It has things like:
#define CS_USED_RBUF 76
{ CS_USED_RBUF, RO, "used_rbuf" }, /* 76 */
That makes it a royal pain to insert or delete a slot.
We should be able to make a macro that does the #define and then builds the
slot.
