I'm guessing from below you mean the src.fedoraproject.org instance?
(pagure.io is the other one, so it's best to be clear which one)

So, src.fedoraproject.org should output fedora-messaging messages and
then notifications acts on those to notify people. 

What settings do you have in https://notifications.fedoraproject.org ?
Also, does this only happen some of the time? or always?

It might be a fmn bug, or something deeper. ;( 

Can you start by filing on fmn:
https://github.com/fedora-infra/fmn/
and we can try and track it down from there?
