I must say that I've seen an increase in reports recently too, and all reporters were using 4.0.1. Unfortunately this bug is hard to reproduce and verify, but maybe 4.0.1 suffers (to a more limited extent, or so it seems from the number of reports) from the same bug that appeared on 3.4.x?
If we ask people to send us the org.openoffice.script.savedState file when they hit the bug, will this help developers to identify the problem?
