OFBIZ-6266 was incorporated in this patch by adding maxPosition to
ModelForm.java so that it could be passed in to
foFormMacroLibrary.ftl#renderFormatSingleWrapperOpen when creating
<table-column/>. foFormMacroLibrary.ftl and foScreenMacroLibrary.ftl were also
extended to include new styles to replicate the format of the original report.
