I tried it a few years back.  It has all the same integration issues (those are independent of the extensions).

The extensions are primarily to support the ZX Spectrum Next: https://www.kickstarter.com/projects/spectrumnext/zx-spectrum-next , I believe.

That has a Z80 inspired. FPGA-based CPU.  It is architecturally similar to the Z180, but with much more practical 8KiB MMU pages
