If you want it to be ALv2 then it is best to:
a) Ask the author to relicense it to ALv2
b) Ask them to sign an ICLA (assuming they are the copyright owner not their 
employer. In most cases it will be their employer.)
c) Ask for an CCLA (if their employer requires it or owns the copyright)

An ICLA is not needed for all contributions, it’s going to depend on the size. 
For large contributions a SGA is needed as well.

If only a) is done then it will be handled differently than if we have a CLA or 
SGA. ALv2 files have different headers that include a copyright line if they 
are 3rd party files.

But for something like this under the BSD license you don't have to relicense 
it, you just need to mention this in the LICENSE file.

There's no official process, other than to ask the person/company to fill it it 
and sent it to the secretary.
