Wrong xref links: main & test classes with same fully qualified name

If you have a main class and a test class with the same fully qualified name
and both contains TODOs, then all the links will be pointing to the xref folder
and none will be pointing to the xref-test folder.
NOTE: New unit test for duplicate filenames should be added.
