I updated the doc patch. It no longer applied after some other changes I made
yesterday so the line numbers didn't match up (I removed all the anchors - the
preference is to use those only when absolutely necessary so it's easier for
people to know how to construct cross-references to other sections). I also did
some light copy editing - if you're OK with it, go ahead and commit it to all 3
branches when you're ready.
