At the time, the JIRA workflow was failing to report successful creation of an issue. Instead, nothing appeared to happen, and the dialog (with submit button) was left on the screen. I know, because this happened to me at the same time. The natural response is to retry the submit, which is what I assume the reporter did. I managed to avoid doing this because I noticed the dups and remembered https://issues.apache.org/jira/browse/INFRA-6926 Rather than leaving the duplicate issues cluttering up JIRA, I suggest they are deleted.
