Hello, recently again the situation is repeated. These errors themselves ... We have added a tap-ctl to monitoring as suggested by Radek.
We have the following observations: - one of the disks has changed status pid = 19919 minor = 16, state = 0 args=vhd:/dev/VG_XenStorage-6df30603-5815-133c-4218-3d49cfca0561/VHD-88291650-4138-403d-9263-973343891a78 pid = 19919 minor = 16, state = 0x2a args=vhd:/dev/VG_XenStorage-6df30603-5815-133c-4218-3d49cfca0561/VHD-88291650-4138-403d-9263-973343891a78 - two other drives has disappeared and tap-ctl does not show them any more.
