Add NS support to DOMElementWriter
ahh, you just obsoleted the chapter on XML processing I wrote earlier in
the week :-(
actually <echoxml> has some more quirks
-no declarations get passed down
-text nodes get merged and come out before/after nested elements.
-UTF8 only
I'm still going to advocate echo with an encoding and CDATA section for
complex XML.
