Thanks. These log messages sound related to the issue:

gnome-shell[15302]: Window manager warning: Impossible to resize screen at
size 5120x2880, error id 8: BadMatch (invalid parameter attributes)
xdg-desktop-por[15805]: Monitor 'Unknown Display' has no configuration which
is-current!

Both of which relate to bug 2060268, bug 2062426 and maybe bug 2066126,
which I can see you are experiencing.

Please apply one or both of the workarounds listed in bug 2060268.

Title:
After screen blank, the displays configuration gets lost

Status in mutter package in Ubuntu:
  Incomplete

Bug description:
I have configured the host screen with two monitors extending the desktop.
They are both configured at 3840x2160, scale 150%, fractional scaling: yes.

After a period of inactivity, the screens get blanked as configured.
When I move the mouse and the screens unblank, sometimes (not always) the
configuration is changed by mirroring the same content on the two displays.
Anyway, the display are still configured as extended in the applet.
Trying to change some settings to restore the desired configuration never
work.
I have to unbplug and re-plug the second monitor.

But sometimes, after unplugging and replugging the secondary monitor,
the session crashes, all the apps are terminated and I get just the
  logon screen.

In other words, gnome is not able to manage reliably the blank/unblank cycle
in a multi-monitor configuration.
I also tried with the AMD built-in graphics card and the behavior is exactly
the same. Therefore I donĀ“t think that the NVidia driver has anything to do
with it.
