Although I have sometimes done the maximize intentionally, most of the time it is completely accidental. I use Ctrl-Alt-T to open a new window and then try to click the "add tab" button a few times to get to my usual working state ... and I miss the button.

Then I double-click on the title bar to maximize it. I do what I need to do with the larger window, then I double-click on the title bar again. The terminal is now 126x27 instead of 132x30. It has been doing this for a while. I'm on a 6.0 kernel now, but I was running with several different 5.x kernels in the past.
