It's not clear to me how this can be a cable or monitor issue when it only happens in Xorg and it happens much more frequently when one particular app is running (see comment #4). Having said that, I will try to find a spare HDMI cable and test.

Sometimes when I plug the monitor in, it comes on, but the resolution is wrong. Sometimes it is actually recognized and configured correctly. Frequently I have to unplug the monitor and plug it back in three times or more before it is recognized correctly.
