Yes, it's very rare, only happened to me twice so far since upgrading to
17.04. Unfortunately, I don't know what triggers it.

Never use terminal tabs so I didn't think to test that. Nor
Shift+Insert, although since even clipboard paste is affected, it's
likely that Shift+Insert is too.

This is a fairly stock Ubuntu-Gnome 17.04 on Xorg.

  pasting into gnome-terminal pastes into a different terminal window

Bug description:
  Ubuntu-Gnome 17.04, latest updates, 28 days uptime

  Open two or more gnome-terminal windows. Select some text and copy it
  (source application doesn't matter, could be a terminal, Firefox,
  etc.). For safety, use text without line breaks, a single character
  will suffice.

  Using middle click, paste into one gnome-terminal, then another, and
  so on.

  When the bug manifests, all paste events to any gnome-terminal go to
  one of the terminals. Right clicking and selecting Paste from the
  drop-down menu is less effective at triggering the bug, but once
  triggered, right-click paste is also affected. Pasting into other
  applications is not affected.

  This bug could cause potentially dangerous commands to be executed in
  a shell, when the intention was to paste them into a file in a
  different terminal.
