FYI, this issue isn't fully gone--I got into this situation today. Logging in with the "Ubuntu" session produced the nautilus-menbar-with- no-launcher issue seen in the screenshots. Logging in with the "Ubuntu 2D" session worked. In ccsm, the Unity plugin was not enabled so I turned it on, and then I also ran unity --reset. After that the problem went away.
