here as well, I recently reinstalled my system to 14.04.02 LTS, Lenovo X240 Thinkpad Laptop, the unlock sim card PIN dialog shows up like described above (randomly?).

Now, every time I resume, I can just Cancel the dialog box, and modem will work.
