confirmed that 2 instances were running. killling them both & starting
a new one fixed this as well as duplicate #1358441 -- but seems like
still a bug that 2 instances got instantiated.

I have a usb Plantronics .Audio 478 headset that I use for conference
calls. Works perfect in trusty, but upon upgrade to utopic the
microphone is not available as an input in the gnome sound preferences
or pavucontrol. There appears to be an entry in alsamixer, and a
source in pactl list.
