Title:
Black screen or xorg-server crash when using hybrid graphics

Status in “xorg-server” package in Ubuntu:
  Fix Released
Status in “xorg-server” source package in Raring:
  Fix Committed

Bug description:
  [Impact]
   * When using a hybrid graphics laptop with an external display connected on
startup, a black screen will show on the external display, or the server will
refuse to start if the resolution is > primary resolution.
   * On nvidia 319 drivers in the prime configuration, the intel display will
stay black.

  [Test Case]
   * Connect an external display to the secondary video card, or use nvidia
drivers in prime configuration.
   * Witness either a blank screen, or xserver failing to start. Correct
behavior is Xserver starting normally, displaying everything.

  [Regression Potential]
   * Patch has been in saucy for a while, no regressions happened there.

  [Other Info]
   * Commit dbfeaf70623a83e1a3f3255c94d52e0e04702837 upstream.
