I started having this issue [both monitors black out, no mouse our keyboard input, still able to SSH in over the network] after upgrading from 11.04 to 12.04. Generally, when I SSH after the freeze/hang, Xorg is taking 100% CPU and I see "EQ overflowing" in the Xorg.0.log.
My setup looks like this. I'm pretty sure the external monitor, or more likely its GPU, is involved because I do not have the problem when traveling without it. I have no reliable way to reproduce the issue. However, it does seem to happen often when I am scrolling a window, such as in a browser or paging in a terminal. Several times the screens have gone blank while I still have a finger on the mouse scrollwheel. Of course, this will not reliably produce a problem.
