Found this reference to the bug on mozilla support site:

In short, it is a confirmed bug for Firefox 16 and is confirmed to be
fixed in Firefox 18.

Do I need to revert to Firefox 15 to address the bug?  This one is
"really annoying", especially as I don't see a manual hack to fix it in
Firefox 16.

  steps:
  disable browser.tabs.warnOnOpen in config
  opened 3 or more tabs.
