The following worked for me. I am on 12.04 LTS.

1. If you have texlive-publishers installed, remove it. (This step is
necessary as otherwise it won't let you do step 3).

2. Follow the steps in this link:

3. Get and install the texlive-publishers package using this link:
https://bugs.launchpad.net/ubuntu/+source/texlive-
extra/+bug/575812/comments/2

This resolves the affiliation bug. In addition, it also defaults to urls
not being displayed with the bibtex in the references.

Bug description:
  Binary package hint: texlive-base

  revtex4-1 included in texlive-publishers (2009-7ubuntu3) does not work with 
version 8.31 of natbib included in texlive-latex-base (2009-7). It requires the 
most recent version 8.31a (see http://authors.aps.org/revtex4/).
  Latex documents using the documentclass revtex4-1 can therefore not be 
compiled on Lucid.
