Create a new application folder (or use an existing one) and make sure that it has at least two pages and at least two apps on the second page. + 2. Close and re-open the Shell Overview mode after this (probably not necessary, but still a good idea to do that). + 3. Click on the app folder and move one of the apps from this folder back to the main app grid. + 4. Re-open the app folder and see how it looks like.
