Did it ever actually show up? I may have added it to the menu myself, cuz I just deleted it from the menu and then reinstalled and it didn't show up. However, I saw another bug saying that the "failed to execute child process" should be changed so as not to mention "killing children" (haha) and that person noted that uninstalling apps does not remove them from the menu.
