I had a quick look at the pactl list output and noticed the following things:
1) You seem to be running the cinnamon desktop. This usually shouldn't matter; but in this case actually it can. So I'm wondering if this is also reproducible from a clean Live-CD of, well, saucy/trusty at this point, and Ubuntu with unity desktop? 2) Only the Maudio card is present; is your onboard card disabled?
