I got this bug, too. On my second monitor, which uses a second xserver,
I discover partial the same problems:

+clicking on folders/files works always.
-toolbar does only work for one "free" single click. after this click: no 
response - also the toolbuttons are not highlighted

My 3 possible ways of workaround:
1) just click the title bar of the nautilus window, then I have on "free" click 
again
so if I want to go 3 folders up, I do:
"title bar <click>", "toolbar: upper folder <click>", "title bar <click>", 
"toolbar: upper folder <click>", "title bar <click>", "toolbar: upper folder 
<click>", 
2) same procedure like in 1), but instead of clicking the toolbar I can also 
move the window with ALT+LMB
3) do not use toolbar, use shortcuts like ALT+UP, because they work

More to say:
There are no problems with nautilus on first xserver / first monitor.

specs:
+Ubuntu 9.10 32Bit
+Linux version 2.6.31-20-generic
+Compiz 0.8.4
+Nautilus 2.28.1
+NVIDIA 7900GS GFX with NVIDIA driver 185.18.36
+dual monitor, two seperate X screens (1920x1200 and 1024x768)
+no xinerama or something (can not drag windows from on to another monitor)

Is it a nautilus bug or a compiz bug or a xserver bug?
