I had the same problem, incredibly slow playback in totem. I had two apps loaded which both use xine backends (note I wasn't trying to play them both at the same time, just had them loaded), totem was using the xine backend to play video, and amarok was using xine for audio. So I looked in ~/.xine/config and tried setting "audio.driver:pulseaudio", it worked - not only can both apps be loaded simultaneously, I can even play both at the same time.
