I had the same issue and the value 0.5 do not work to me. The Print Setup dialogue mentions that these values are in mm. i.e. 0.5 mm from the top is very, very small. After I changed the values to 5.0 for both the header and footer are back again. Is there an issue between measurements (US vs metric)?
