Any chance you're running a private IP range and that the internal
DNS server doesn't have an MX entry for your domain?

But that's not what I'm seeing. Users sending messages from outside
my WAN will fail the MAILFROM test whether they have mydomain.com
as their SMTP server or not. I'd understand if the test checks if
the Mail From address matches the SMTP server but even that's not
case.
For a test that has a default weight of 12 I must be missing
something obvious.
