Maybe the server doesn't like having a Monitor-S-video option without the same for other outputs, it thinks only this monitor should be used but fails because of the "ignore" option. You should try adding similar monitor-VGA-0 and monitor-LVDS options and new Monitor sections for VGA-0 and LVDS (only an identifier is required, and of course you don't want an ignore option there).
