https://bugs.freedesktop.org/show_bug.cgi?id=323 describes a patch to use EmulateWheel and the 3rd button of the mouse concurrently. Although it was added to the xorg-respository in autumn 2004 it seems to be still missing in debian xorg version. At least it doesn't work for me and a quick search in the debian-xorg trunk showed that it isn't there.
