This is particularly noticeable with the "_" character in Andale Mono at large sizes. This is fixed in the latest upstream xterm; looking at the upstream changelog I believe the relevant change is patch #180: make height of TrueType fonts match ascent+descent (patch by Keith Packard). That upstream version also makes underlining work with Xft rendering, which would be nice to have too.
