On the second head, the image starts again from the top left corner. An example of where this is a problem is when a user wishes to use XPlanet to display an image of the Earth as a desktop background. Only half of the image can be seen. An interesting side-effect: The X Server _is_ actually supplied the entire image. If you run a program such as aterm or eterm with transparent backgrounds enabled, that program will show what is supposed to be on screen at that location on the second head. I had heard through the grapevine that this bug was supposed to be fixed in XFree86's post-4.0.3 CVS, and I had never personally tried compiling 4.1.0 to test.
