I've experienced the very same here (when trying out the Debian derived Knoppix live-cd distribution). After I made the drivers directory contain mga_hal, it worked, too. It would take a moment to produce it (especially since I just tried the cd before giving it to someone else), but if it helps, I could send another logfile. (My Display is a TFT (with DVI), so it has rather low rates, but still XFree seems to send 1xx kHz HSync unless it has mga_hal. With the pre5v1 packages, mga_hal does not seem to work for me either... I have switched to fbdev driver for the moment, but would be happy to help if I can aid the resolution of this bug.
