Well, I think that the concept of pure CDDs, entirely built from official the Debian repository by simply selecting packages and preseeding, and possibly integrated in the official Debian releases, is the Right Thing to do. However this is not always possible in the real world. As distribution, 64 Studio 2.0 (released a couple of months after etch got out) is quite close to a pure CDD, because being a DD I had care of uploading all packaging work directly to Debian, or submit patches as needed. The only outstanding piece package which is not in Debian is the Linux kernel, which needed to be patched for audio realtime work. I and other members of the Debian Multimedia Team have once tried to get this realtime flavor of the kernel in Debian/etch, but we didn't have enough man power to assure the Debian kernel team that we would have supported such a flavor for the whole life-time of etch. We are now about to release 64 Studio 2.1, which will be basically a 2.0 plus backports of audio-specific applications, hence the new release will be less "pure". I believe this is a general problem with pure and non pure CDDs, as everybody has different release schedules. Ciao!
