Yes, I've been running apt-cacher-ng of various versions for around a decade, and probably have to babysit it about once a month (my nightly machine rebuilds from scratch fail and I have to go in and do a variety of maintenance items on the admin web page).
One of my two goals with apt-cacher-ultra has been to never have to do manual maintenance, and while I only have ~4 months under my belt, so far it's been maintenance free. The other primary goal was for it to continue working when upstream is down. Our Ubuntu respins got no help from a-c-ng when there was that massive DDoS of their infrastructure.
