I used to use xsane almost exclusively for scanning to PDF, but it only does single pages. Now I mostly use either simple-scan or naps2. I find that simple-scan seems to work better for black-and-white scans with pencilled annotations, while naps2 is a very flexible utility that allows pages to be rotated, or re-arranged by dragging and dropping. When it comes to selecting individual pages or combining multiple PDF files, I use pdftk.
