For important/expensive items some sellers require the user to have completed a certain number of transactions successfully. If not they wait until the check clears before shipping the item. I think most buyers also look at the record of the seller and make sure he doesn't have any negative feedback. For buyers, I think ebay automatically insures items of up to $500.
