I suppose it could be at your place, but while I haven't dealt with anything but the local ISP's here in the USA, it has always been considered as part of the deal. I have a 10 megabit connection and my telephone from the local cable company at nominally $110 USD/month. And the telephone is no charge long distance anyplace in the lower 48 states. That quasi-free LD is handier than sliced bread as this is a pretty big place compared to NZ. I find I often have to check where I'm calling to make sure the time zone there is business hours for them since we have 4 hours diff to kalipornia from here.
