In my experience, with the Debian packaging, this is usually caused by exim conflicting with itself. I've never figured the problem out entirely, but it is exacerbated by having exim listen on more than one port (-oX 587:465:25) -- To UNSUBSCRIBE, email to debian-user-requ...@lists.debian.org with a subject of "unsubscribe".
