I had seen this a few times myself, bug #545179ยน might be the culprit.
In general, any package shipping a library that /sbin/init is linked
against must run "telinit u" in the postinst to ensure that the root
filesystem can be unmounted on shutdown.

Too recent, I still have:

ii  libc6                     2.9-24

The bug had been introduced in version 2.9-23, according to the bug
submitter.


Found the problem, but not why it happened: all the K|Sxx<name> in /etc/rcX.d have the wrong xx's!

Have no idea how it happened.
