The problems are probably stemming from your browser's Flash plugin. I had similar problems - everything worked until the first time a Flash object in a page (like Youtube) tried to play audio. After that no other applications could access the audio resources they needed and even closing the browser didn't help. I've no handy solution, since after way too much messing around with configuration files, I took the easy option and killed off PulseAudio.
