I don't know the ultimate cause, but I have a suggestion on what to do if it happens again (other than just pulling the plug). Manually run each script in /etc/rc0.d with the parameter "stop". This is what the shutdown command should be doing and you may see an error indication that gives a clue. If not, at least you may avoid mangling your filesystem (or some data on it).
