Ext2/Ext3 filesystems have a percentage of space reserved
for emergencies. You can change the percentage with
"tune2fs -m number". You can do this safely at any time,
even when the filesystem is mounted.

IIRC, the default is 5% for Root use.
728G 699G 0 100% /
but that should be about 37G as per your df -h posting (half of 10% of 728G)
I suspect that is your issue.
