The following problem happened while doing an 'apt-get upgrade -u': The first run of apt-get upgrade crashed with a segmentation fault after downloading all the packages and while unpacking and setting them up.
After the segfault I run the command again, the result was no output on the terminal and apt not returning, hitting CTRL-C did not cause the process to be killed. kill -9 did not kill the process either. I could not run apt-get form a different terminal because the process kept the package database locked. The STAT column of the ps command had a value of 'D', the TTY was '?', and the process was taking no cpu time.
