Could this, by any chance, be a SCAM issue (SCAM: "SCSI configured automagiclly")? If yes, try to disable SCAM support in your SCSI configuration utility (or SCSI BIOS setup or whatever it is called). And make sure that every SCSI device has a unique SCSI id.
